事故調(diào)查組結(jié)合消防和專家意見以及事故調(diào)查實(shí)際情況，認(rèn)定事故直接原因是由于1-己烯灌裝重量設(shè)定值設(shè)置錯(cuò)誤，導(dǎo)致原設(shè)定充裝140kg的物料桶灌裝至130kg左右時(shí)1-己烯氣液相溢出并發(fā)生噴濺產(chǎn)生靜電釋放發(fā)生閃燃。
Based on the opinions of the Fire and Rescue Detachment and experts as well as the real conditions of the investigation, the Accident Investigation Team identified that the accident was directly caused by the inaccurate set value of filling weight of 1-Hexene, and consequently when the material barrel originally set to fill 140kg was filled to about 130kg, 1-Hexene gas spilled at liquid phase and splashed, generating electrostatic discharge and leading to flashover.

Existence of rule-breaking operation. The Company violated the provision of Article 4 of the Notice on Further Enhancing the Safety Management of Road Transportation of Hazardous Chemicals in Summer (H. A. J. G. W. H. [2011] No. 105), “the handling operation of hazardous chemicals is forbidden from 8:00am to 4:00pm in summer”, and the unloading and filling operation of hazardous chemical 1-Hexene at the forbidden time frame indirectly caused the accident.
